There's going to be some major burnout if they try to squeeze four ladder matches onto the card.
I think the smartest thing to do is just do one for the men and one for the women where the contract is either open-ended and the winner can challenge either champion; or specifically states that it's good for a shot against the champion of whichever brand the winner is on.

As I said in my SummerSlam thread, I kinda think Miz is gonna win it. Then he can steal the title off Bryan should he win it again. And I'm thinking that is why Cass is involved with Bryan now, so they can save the Miz feud.

What would be cool too is if they had an NXT Ladder match on the show, and the winner receives a contract to be on Raw or Smackdown.

Main event Guys like Braun, Brock, Roman, Rollins, AJ, Nakamura, Cass, Sami and KO, maybe Taker, Tag Teams, Cruiserweights and Women's title matches can fill out the rest of the card

Yup. I'd maybe add a tag team one for that "FIRST EVER" bit of appeal, but three on the show might be too many.

I honesty prefer the unpredictability that a single MITB match brings. Will they cash in against the Raw or Smackdown Champ each week brings intrigue to the roster. Every time either champ gets beat down could be the moment that the guy/gal makes that run to the ring. I say go with two one for the men and one for the women. Make each match have 8 guys/gals respectively and let the best man/woman stand tall at the end.

If they’d actually make it so that the winners could cash in on either champion then yeah that would be really cool to be thinking about it every single show. But knowing WWE they’d likely just say that the winner gets a contract for the championship of their brand.
