Honestly, why the  does all of America care every time this girl ing sneezes? She's not attractive, and even if she was, she's just a ing celebrity. I'm convinced the Vanity Fair photos and the other "leaked" photos were a planned publicity stunt/career move anyway.
Britney did the same thing ten years ago. When she decided to make sure she wasn't pigeon-holed as a wholesome role model, all of a sudden, there were provocative photos of her in Rolling Stone & everywhere else.
It's actually a very common strategy for young actresses and singers, as a way of upping their sex appeal and pushing back against the people who push them to become this high-and-mighty example to the youth. They concoct a reasonably safe "screw up" (such as provocative photos that -- OOPS! -- somebody must have leaked online) that is harmless enough to do no real long-term damage, yet alarming enough to make people stop categorizing them as a role model or a "tweener."
You can come up with a lot more examples, like Christina Aguilera's skanky years, Jessica Biel's topless photo shoot for Gear, or Lisa Bonet doing Angel Heart as a way of rebelling against her Cosby Show image.
I think Miley Cyrus (and, to be fair, her people) are acutely aware of what they are doing, and they planned all along to get some racy photos out there... and then to act flabbergasted and "embarrassed" when they went public, claiming innocence in the matter. Show business is pretty Fed up sometimes.
