you can switch their heads by boiling them (stick the head in hot water for a bit and the plastic becomes soft enough to pop off the peg)
Thing is, some figures like Rick Martel aren't made this way and their head is attached to the body from a peg going into the body so when you cut it away, you're left with a crappy little stump
Figures like LOD Hawk are great for popping the head off and making a new figure...

Yeah on Hasbro some heads are easy to switch and some are molded onto the torso, so they really can not be switched unless you saw off the head and glue it on to another body and sculpt around the neck to make it look normal. As far as hands it can be done (see adrione's custom Kane - wfigs.proboards48.com/index.cgi?board=Customs&action=display&thread=1156156650) and basically it's done by chopping off the hands with and exacto knife, gluing on the hands you want with some super glue or epoxy glue then sculpting around the joint to make it look smooth. It sounds harder that it is, but it does take a couple of tries to do it.
