F**kin' 'Ard, I believe it was Insurrextion 2002 the ropes broke, I remember watching it live. I think Taker fell onto the ropes after a back suplex, and it gave way.

The WWF vs. Alliance angle. There was so much money to be made and WWE completely made WCW/ECW out to look like useless jobbers. And no, sending Steve Austin to the Alliance doesn't count in the "The Alliance didn't look too bad," argument. Just a really, really badly executed storyline considering the natural money-making feud WWF vs. WCW vs. ECW could have been.
