Dr. Death was a bad choice because he was rarely seen in WWF before the figure release and even though it was released around the time it came out he wasn't in WWF very much at all around that time. Other than Brawl for All I never did see him on TV. There is a trading card of him pressing someone over his head but that's all I've seen of him other than BFA and I watched the shows all the time and don't remember him in regular ring competition.
The release of the figure almost seems like a Alberto Del Rio "a big deal fairly quickly" thing in that they wanted a figure of Dr. Death to coincide with his return against Bart Gunn. Problem was people didn't know much about him in WWF and there wasn't even any vingettes/promos for him.
Another problem was the accuracy of the figure. Dr. Death wore a red singlet with white criss cross like straps surrounding it. Not the red shorts and white tank top type singlet the figure had. Jakks has done some inaccurate toys before from a horrible Rock in 1999 with half short that say "Rock" on one leg, a Savio Vega with plain white shirt as some of my least favorites but Dr. Death was pretty poor and I'm glad for Jakks sake it didn't sell well.
I wonder if Dr. Death knew about the poor sales. It would be too bad if he did because he deserved better. Jacqueline was another poor seller and she knew about it. I think it was Heat before Survivor Series and she said she went to the store and "they're all out of Rock, they're all out of Stone Cold but they have plenty of Jacqueline dolls." It must be embarassing seeing a load of your toys not selling in the store.

Dr. Death and Jacqueline also warmed shelves particularly badly because they were released in main lines, which EVERY store got in abundance, rather than, say, one of the K-Mart exclusive lines. Dan Severn had two different releases and he had less TV time than Dr. Death, and was a much more plain figure. But because his figure wasn't MASSIVELY over produced, it wasn't a problem. Jacqueline was released in a line with Edge's debut figure, Austin, Rock, Undertaker, and Triple H. Want to guess which ones from every case sat?

Like previously mentioned, BCA Dr. Death & Jacqueline. As far as TTL, I want to say Unchained Fury Series 1 Ric Flair. And for RA, RA7 Randy Orton and RA15.5 Simon Dean were pretty bad.

The Dr Death figure was just plain ugly too. Way too fat. It seems like every Jakks wave of bonecrunching figures always had a WTF figure in it. Like in Series 9 the suit on Vince made him look like a blimp.
From their classic line I saw the Horsemen ones and some of the other 2 packs on TRUs shelves for quite some time. Think they have cleared most of them out now.

Thank you! I instantly thought of his figure but couldn't remember what series he was from. CS 27 Barbarian and CS 23 Bob Holly were around for a while. But yeah CS 5 Sheik was around for a looooooong time.

Weird because I remember seeing Dr. Death like twice in store & ended up finally picking him up at a boot sale in 2000.
I remember Woolworths having ADR13 La Resistance forever.
