I know a few different figures come with ladders, and I'm pretty sure a Rollins came with the MITB briefcase, but how are you supposed to suspend the belt/case for a Mattel ladder match? Is there an "official" way or do you just use some string or something?

A cheap and easy way is tape a long string to the roof above your ring, bend and tie a paperclip to the end of the string that you can hook a belt or brief case to.
Looks better to me than any of the gimmicky structures that they've released.

They have made 2 different types of playsets for the mitb set up. Search ebay for them.

When I first got into mattels I got the flexforce mitb and I've been using that ever since.

Basic ring Money in the Bank and TLC playsets. Flex Force TLC playset. Create-a-Ring playset. Upcoming authentic scale MitB playset (someday soon maybe?).
Or do what I did and tie a string from ceiling fan, attach top scaffold from MitB playset to said string, and dangle briefcases/championships from it.

If you have a ceiling fan with a dangling cord you can hang a string or chain from it.. I feel like a small-link chain looks cool for hanging belts in ladder matches.

Real men use dental floss and a paperclip.
That's what I did back in the day. Dental floss was used for ladder matches, Spidey's webs, Batman's ziplines , etc

No disrespect to dental floss. Even today, I keep a roll of heavy gauge fishing line in my utility closet. Perfect for hanging action figures and "flying" vehicles from the ceiling.
