I always thought of that as a really creative moment to do the blade job.
It also has to take some skill because you're entire bdy is moving while you're putting the blade to your skin.
It's usually not that obvious, that's just a bad camera angle with a picture taken at a bad moment.
Either way, non "smart" fans wouldn't even know that's what he was doing anyway and they're definitely the majority.

He always does it at that moment. Watch SS 2003, right before Christian tosses him into the ring post. The camera pans down, sees him, then goes back up.
