to the best of my knowledge,no, Ax and Crush never teamed together,at least on TV. it was always Smash and Crush that they showed.
Ax just left after Survivor Series. they never mentioned his leaving,or why,or anything. he just wasn't there anymore.

Is Crush actually a recognized WWF Tag Team Champion? I know he defended it under the "Freebird Rule", but I've never known if he's a recognized holder of the title. Yes, he is recognized as champion. From his WWE.com bio

LOL at WWE revisionist history. In 1990 the Freebirds Rule would have never been mentioned and probably not even thought of. I guess sometime after the WCW purchase someone noticed the similarity (a three man team and a three man team) and went with the Freebird Rule. Probably Michael Hayes.
