Definitely had a pre-tape somewhere in there.
God, that whole thing was awful.

That segment was rad, I would have loved if after he took all that beating, he did the whole sit up thing instead, even with the suit and all, that would have been badass

I should imagine it was pre-recorded so that, as soon as he went backstage there was a small team of people to sort him out. One person with his glove, another with his elbow pads and another for the make-up.
I would have had him appearing in the ring as others have said, but I think what they did worked pretty well.

All he had to do is remove his suit and add a glove and mask and eye paint. which is easy enough to do in the time he was in the ambulance.

When I initially heard of the split personality gimmick I thought it might be a way of introducing a new wrestler into the Kane character so Glenn Jacobs can retire... Just a thought... Might still be the direction they head in.

Also was probably wearing all his gear under his suit. But he still had remove his suit and then put on his glove, boots, mask and paint. That would've taken longer than the short time he was in the ambulance. They pretaped the ambulance segment and spliced it. So when Kane got backstage after the attack, he just got his monster gear on. The part they show of Kane getting loaded in the ambulance was taped earlier in the day. When the camera comes back and the ambulance stops is coming back live and out comes monster Kane. You could tell the second part was live bc Kane's wig wasnt exactly on right and they probably were a bit rushed.

That would've taken longer than the short time he was in the ambulance. He was wearing his boots with the suit. And was likely wearing the wrist tape and elbow pads underneath too. All he really had to do was quickly take the suit off, rub some paint on his eye area, put the mask on, put the glove on... voila. The segment was absolutely incredible.
