The only one on one matches I can think of off the top of my head between the two are WrestleMania14, The Inferno Match, the title match that ended in a no-contest, and the WrestleManiaXX match. Kane has never come out on top in any of them. Did I miss any?

They also fought one-on-one at SummerSlam 2000 (I think?) but the match never officially started. Undertaker unmasked Kane and Kane ran out covering his face.

And almost tripped over Taker's bike whilst doing it. Yes, Kane has beaten Undertaker numerous times. He beat him before the Inferno match (Undertaker practically laid down though, as he swore he would never hurt his little brother) and then a few other times before 2001 when they joined up. They never really had a big match on PPV against each other though. Usually they wrestled on RAW/Smackdown! and often in triple threat and fatal four ways. Kane's victories were never that glorified or talked about.

Didn't Kane beat the Taker in a Buried Alive match, after which Undertaker returned for revenge? This was only a couple of years ago.

They also wrestled in the WWF Championship tournament at Survivor Series '98, but Undertaker won that one too.

Didn't Kane beat the Taker in a Buried Alive match, after which Undertaker returned for revenge? This was only a couple of years ago. The match was McMahon vs. Taker. McMahon had been a thorn in Takers side and Taker finally one a chance to pick who he would face in what type of match and he "surprisingly" (in quotes because it was obvious he would) chose McMahon in a Buried Alive match. The match was brutal, McMahon bladed way to deep and was a mess, it culminated with Kane coming out throwing Taker in the plot covering it up and McMahon winning this was the death of ABA. This led to the WMXX match and the reappearance of the deadman gimmick.

Kane also did basically the same thing at 'Rock Bottom' when Taker faced SCSA.

Undertaker and Kane also fought each other at the 1998 Judgment Day PPV, but it was a no-contest...

Kane defeated the Undertaker on a Monday Night Raw in June of 1998. This was a number one contender match with the winner facing Stone Cold Steve Austin at King of the Ring 1998 (better known as the night the Undertaker threw Mankind off the Hell in the Cell). Kane won via Tombstone after a distraction by Mankind. I thought they had an awesome build up, Kane locked Austin in a casket, had "blood" fall from the ceiling and drench him, Kane spoke for the first time via voicebox saying if he did not win the title against Austin he would set himself on fire. Kane went on to win the WWF title in their first blood match after Undertaker busted Austin open, only to lose it back to Austin in a rematch only 1 day later on Raw.

The only time I remember is the time stated above (before KOTR98).

Kane has beaten Taker before such not on the higher status Taker has. Kane really should have won that WM contest with the multiple piledrivers.
